This tumor affects the duodenum periampullary regions or the ampullary. Many of the <strong>D</strong><strong>uodenal Cancer</strong> could be easily found at an early stage. <p>Earlier<strong></strong><strong>D</strong><strong>uodenal Cancer</strong> could be easily found out by the endoscopy without identifying the symptoms of the disease. A new type of cancer found recently that are found on the <a href="http://duodenal.org/duodenal-bulb/">duodenal bulb</a>. By using the gastrointestinal radiography, the cancer could be easily detected. In this article, the radiographic, clinical and pathologic detection of cancer is discussed. <span id="more-97"></span></p>
<p>Case Report of patients affected by <strong></strong><strong>D</strong><strong>uodenal Cancer</strong></p>
<p>An 85 year old man observed different health problems of iron-deficiency anemia, guaiacpositive stool and aporadic pain in the lower abdomen. The doctors treat the patient by using H2 receptor antagonists. The gastrointestinal radiography has found out the stalkless and a small bit of lobulated polyp at the lower part of the duodenal. This ulcer has the diameter of 1.3 cm that was distinct from pylorus. Later on doctors found a 1.5 cm polyp by doing endoscopy of the duodenal bulb. By performing endoscopic biopsy has revealed out a papillary adenocarcinoma.</p>
<p>During the surgery, a tiny lobulated polyp of 1.5 cm is observed in patient duodenal bulb. The treatment of the patient is done by using gastroduodenectomy . After the pathologic examination of the cut edges, the treatment has brought out the papillary adenocarcinoma that was present at the bottom of the duodenal bulb. This ulcer was detected as a mucosa, whereas the lymph nodes are not affected by this cancer.</p>
<p>After the treatment, the lesion was known pathologically as the<strong> </strong><a href="http://duodenal.org/duodenal-mucosa/"><strong></strong><strong>D</strong><strong>uodenal </strong><strong>Mucosa </strong>Cancer</a>.</p>
<p>Discussion:</p>
<p><strong></strong><strong>D</strong><strong>uodenal Cancer</strong> is uncommon, but yet it is responsible for small cancers from 45% to 65%. The tumors are found in duodenum due to the adenomatous polyps that was present beforehand. This cancer is generally found in periampullary regions or ampullary regions that are near to the duodenum. Sometimes, the tumors could also occur in other body parts.</p>
<p>The symptoms of the <strong></strong><strong>D</strong><strong>uodenal Cancer</strong> are nausea, vomiting, epigastric pain or weight loss. Most of the diagnostic patients that have <a href="http://duodenal.org/duodenal-cancer/"><strong>D</strong><strong>uodenal Cancer</strong></a> suffer from poor prognosis that would reduce the survival rates by 5 year that ranges from 20%-40%. The other factors that hamper the survival rates in patients could be the presence or absence, depth of invasion, symptoms durations and distant or nodal metastases.</p>
<p>The advanced stage of the <strong>D</strong><strong>uodenal Cancer</strong> is found as the ringe lesions, ulcerated or polyploidy. Earlier reports have not detected the <strong>D</strong><strong>uodenal Cancer</strong>. These could only be found due to the radiography and by endoscopy. Other tumors could be found by the radiography referred as the polypeptide lesions. Both the type of the polyps looked smooth and rounded in shape. A common type among all these is the benign tumors. The barium studies have showed that the polyps having more than 1 cm diameter were the ulcerated or lobulated lesions.</p>

This ulcer can be distinguished as two. The one which is present inside the human stomach is known as gastric ulcer or the <strong>duodenal stomach ulcer</strong>, and the one that is present in the duodenum is known as <a href="http://duodenal.org/duodenal-ulcer/">duodenal ulcer</a>.  The cause of these ulcers can be different but researches have found some new reasons also. As in the past the causes of these ulcers are believed to be the lifestyle of the people and the diet they take.</p>
<p>Then after some researches it was found by the doctors that the reason behind the causing of them are stomach acids known as hydrochloric acid and pepsin that facilitates their formation.  And nowadays researches show that 80 % of gastric ulcers and 90 % of the duodenal ulcers are caused due to a bacterium of the name Helicobacter pylori known as <a href="http://duodenal.org/duodenal-polyp/">h. pylori</a> in short. All the above are the cause of development of ulcer, but H. pylori is said to be the major cause.  Now let us know about the causes of the development of <strong>duodenal stomach ulcer</strong> in more detail.<span id="more-79"></span></p>
<p>There are certain things that must be considered to be the cause of the development.  The first as we had known above is the helicobacter pylorus, which weakens the protective mucus of the stomach through release of some substances which makes the mucus become more prone to the harmful effects of the stomach acids and also to produce greater amount of acid.  Next cause is smoking. People who smoke regularly may get ulcer as its chances are increased also if there is an existing ulcer then smoking makes its healing process slow there may even chances of reoccurrence of the <strong>duodenal stomach ulcer</strong>.</p>
<p>Caffeine is also considered for the cause. Alcohol, which at many times found to be a reason behind some diseases is sometimes considered to be one of the causes though not found to be exactly but the people who consume alcohol get a disease called cirrhosis, and <strong>duodenal stomach ulcer</strong> is mostly common present in these patients.  Stress is today not considered to be direct cause of <a href="http://duodenal.org/duodenal-stomach-ulcer/"><strong>duodenal stomach ulcer</strong></a>, but the people who have got ulcers do tell about the increase in their ulcer pain due to stress.</p>
<p>Physical stress may cause the development of ulcer. When stomach is unable to fight the acids like hydrochloric acid and pepsin then it may facilitate the formation of an ulcer. Another reason is NSAIDs that is non-steroidal anti-inflammatory drugs such as aspirin, ibuprofen etc. which can make the stomach become defenseless to the damaging effects of pepsin and acid.  Now let us know about the symptoms of ulcers. Though every affected person shows different symptoms of ulcers but there are some which can be mentioned. The common among these symptoms is a burning pain, which is felt in the abdomen between the breastbone and the navel by a patient.</p>

			<content:encoded><![CDATA[<p>In the Surgical Service of Grady hospital, two cases of <strong>duodenal hernia</strong> were observed within a period of three months. According to Moynihan classification, one of it was present at the right side, whereas the other was present at the left side. Both cases were observed clearly to find the symptoms. The first case was diagnosed correctly only by facing a fatal termination. In the second case, it was easily diagnosed by using exploratory laparotomy.<img class="alignright size-full wp-image-76" title="Duodenal Hernia" src="http://duodenal.org/wp-content/uploads/2011/11/Duodenal-Hernia.jpg" alt="" width="154" height="154" /></p>
<p>The <strong>duodenal hernia</strong> first description was found by the Klob in 1861 that is available in literature. He got a case that has a hernia at the right side. In year 1857, he depicted the possibility of the duodenal hernia that was formed in the different folds.</p>
<p>In 1906, Moynihan has presented the detailed report of the cases of <a href="http://duodenal.org/duodenal-hernia/"><strong>duodenal hernia</strong></a>. In the report, the hernia was explained clearly. The embryologic and anatomic interpretations of both the cases have been confirmed by using Moynihan observations that were carefully studied in both cases.<span id="more-75"></span></p>
<p>The intestinal canal was represented in its fourth week of intra uterine life. The two elongated tube was connected all through the length by perotoncum tube. The dorsal bulge formed that result in building stomach and the primitive mesenteric got connected with it. The abdominal wall has a peritoneum coming out.</p>
<p>During the sixth week, the special arteries have provided the three segments that were noticed. These segments help in developing the duodenum and stomach, whereas the celiac axis is present in the mesentery. <p>According to Moynian description of the <strong>duodenal hernia</strong>, the duodenal jejunal is classified int onine fossae. However, only two of them have only given teh detailed description.</p>
<p>The <strong>duodenal hernia</strong> is rare in the remaining fossae, but in case of duodenal hernia, a proportional frequency has resulted having a fossae that could be due to the following reason.</p>
<p>1)         In the case of <strong>duodenal hernia</strong>, superior duodenal fossae are present at the left of the <a href="http://duodenal.org/duodenal-bulb/">duodenum ascending portion</a>.</p>
<p>2)         In 80 % of <strong>duodenal hernia</strong> cases, inferior duodenal fossa are present in between the third and fourth vertebra by lying at the left ascending portion.</p>
<p>3)         At the ascending limb of duodenum, posterior duodenal fossa are present behind the upper part.</p>
<p>4)         At the base of transverse mesocolon, duodenal jejunal fossa is present.</p>
<p>5)         The inter-mesocolic fossa is present at the transverse mesocolon base.</p>

<p>In a body, when the food enters as soon as it has been taken in mouth and chewed, the process of digestion begins. The digestion process is a complex and critical process. When the food reached the digestive system then the gastrointestinal tract or the digestive system got stretched to the esophagus from the mouth.</p>

			<content:encoded><![CDATA[<p><strong></strong><strong>Duodenal Carcinoid Tumor</strong> occurs rarely, and till today, there is no serious case were observed that required the <a href="http://duodenal.org/duodenal-switch-surgery/">surgical treatment</a>. When <strong>Duodenal Carcinoid Tumor</strong> was pathological diagnosed for the 24 patients, then around eighty percent of them were found to be measuring only 2 cm diameter. Around 85% of the patients suffer from the submucosa and mucosa. <img class="alignright size-thumbnail wp-image-59" title="Carcinoid Tumor Diagnosis" src="http://duodenal.org/wp-content/uploads/2011/12/Carcinoid-Tumor-Diagnosis-150x150.jpg" alt="" width="150" height="150" /></p>
<p>In a case of 13 patients, their lymph nodes were examined to find the <strong></strong><strong>Duodenal Carcinoid Tumor</strong>, but only the lymph node metastases were observed in 7 patients. Among them, only 2 patients have tumors having size smaller than 1 cm that was like the size of submucosa. After the continuous period of 46 months, the disease survival rates exceed to 100% and only two patients have observed the reoccuring the regional lymph nodes. During the time, no patients were found suffering from carcinoid syndrome or distant metastases.<span id="more-49"></span></p>
<p>The authors of the above case have done study that in the patients suffering from tumors and found that the <strong>duodenal carcinoids tumor</strong> are inactive. However, the lymph node metastases could not be determined on the basis of the tumor size and their effect on the survival rate has not yet clear.</p>
<p><strong>Duodenal Carcinoid Tumor</strong> were treated by using the endoscopic excision that was found as an appropriate method of the primary stage treatment for the tumor having size less than 2 cm. The full thickness excision were operated in the tumors having size in between 1 cm to 2cm.</p>
<p><strong>Duodenal Carcinoid Tumor</strong> that have size of 1 cm could not be removed completely at the first treatment of endoscopic. Therefore, before using the endoscopic resection, they have been examined under the endoscopic ultrasonography, so that it does not affect the areas beyond the submucosa.</p>
<p><a href="http://duodenal.org/carcinoid-tumor-diagnosis/"><strong>Duodenal Carcinoid Tumor</strong></a> that have size more than 2 cm needs to be managed properly as it seems to be problematic. However, this type of tumor has been treated by using the regional lymphadenectomy and full thickness excision. The Lymphadenectomy could even be performed as the lymph nodes are a major cause of these tumors.</p>

<p>According to some authors, <strong>Duodenal Carcinoid Tumor</strong> that are larger than 2 cm needs to be treated using Lymphadenectomy along with the lymph nodes on the basis of below locations:</p>
<ul>
<li>If the anterior present, to the inferior vena cava, pancreatic head or the posterior present at the duodenum.</li>
<li>Portal vein and posterolateral present at the bile duct.</li>
<li>Anterior present at the common hepatic artery.</li>
</ul>
<p>During the surgery time, the imagining studies that have found the abnormal lymph nodes should be cutted down by noticing the primary tumor size. This is because, only small history is known about the tumor apart from the nonoperative management and lymph node metastates. The lymph node patients were given the continued radiographic surveillance rather the primary tumor size.</p>
<p>The periampullary and ampullary <strong></strong><strong>Duodenal Carcinoid Tumor</strong> needs to be observed minutely as they are alike histologically, clinically and immunohistochemically compare to the <strong>carcinoid tumors</strong> that are present in other areas, in duodenum. These types of tumour were treated in a different way and are unpredicted.</p>

<p>Know your digestion and gut</p>
<p>When food is, consumed then it reaches into oesophagus in the stomach. The stomach digests the food by making acid. Once the food gets mixed properly then it reaches into duodenum (foremost part of the small intestine). The food then mixes well with the enzymes after reaching duodenum and small intestine. The role of enzymes is to break the food so that the body could absorb in easily.</p>
<p>Explanation of Few Terms:</p>
<p>•           <strong></strong><strong>Duodenal Ulcer</strong>: It is one of the most common forms of <strong>peptic ulcer</strong>.</p>
<p>•           Peptic inflammation: This inflammation arises due to the stomach acid. It could be either in lower oesophagus or duodenum.</p>
<p>•           Peptic Ulcer: This ulcer also results due to stomach acid. The ulcer could be present were the tissues are exposed or where the guts lining were damaged.<span id="more-29"></span></p>
<p>Cause of <strong></strong><a href="http://duodenal.org/duodenal-ulcer/"><strong>Duodenal Ulcer</strong></a></p>
<p>It is a normal stomach behavior to produce an acid that helps in food digestion and killing bacteria. As this acid is corrosive in nature, so it produces some cells along with the duodenum that produces mucus. All together these cells and mucus act as a barrier for the stomach lining. There is always a barrier between the mucus and acid. In the case, when the balanced gets disturbed then it results in damaging the stomach lining that causes <strong></strong><strong>Duodenal Ulcer</strong>.</p>
<p>This <strong></strong><strong>Duodenal Ulcer</strong> is caused due to following:</p>
<p>•           The infection is caused by Helicobacter pylari</p>
<p>•           Sometimes anti-inflammatory drugs are also responsible</p>
<p>•           Other reason could be Zollinger-Ellison syndrome, or stress, smoking or drinking are rare causes.<strong></strong></p>

<p>•           One of the common symptoms is the pain that appears in upper abdomen.</p>

<p>a)         Bleeding Ulcer: This type of ulcer could case life threatening bleed.</p>
<p>b)         Perforation: In this type, duodenum walls get affected by the ulcer.</p>
<p>Test to identify <strong>Duodenal Ulcer</strong></p>
<p>•           Endoscopy: This test when performed confirms the <strong>Duodenal Ulcer</strong>. In test, a thin telescope is passed in the abdomen viewing by doctor.</p>
<p>•           Test for detection of H.pylori bacterium: This test is preformed commonly to find the <strong>Duodenal Ulcer</strong>.</p>
<p><a href="http://duodenal.org/duodenal-switch-surgery/"><strong>Duodenal Ulcer Treatment</strong></a>:</p>
<p>1) Acid suppressing medication: The medicine uses is a proton pump inhibitor. It helps in reducing acid.</p>
<p>2) The ulcer that is caused by H. Pylari whose main objective is to clear the infection. It is important to clear the infection fully otherwise it has the chances of returning again.</p>
<p>3) The ulcers that are caused by anti-inflammatory drug needs to be stopped consuming.</p>

			<content:encoded><![CDATA[<p><strong>Duodenal Adenocarcinoma</strong> is a very uncommon disease, which does not show any previous history of its presence and occurrence. Therefore it is still a matter of discussion and the survival percentage of its patients and the ways to tackle this disease is still not exactly known. <img class="alignright size-medium wp-image-22" title="Duodenal adenocarcinoma" src="http://duodenal.org/wp-content/uploads/2011/11/Duodenal-adenocarcinoma-300x259.jpg" alt="" width="180" height="155" /></p>
<p>The symptoms of <strong></strong><strong>Duodenal Adenocarcinoma</strong> disease many times confuse doctors. Thanks to the development of advance endoscopy and radiology that has helped in unearthing many of these type of cases. The only means by which potential cure can be achieved is through definitive surgery and the node-negative patients become much better with this prognosis.  As it is told above it is an uncommon disease which presents itself in less than .4% among all gastro-intestinal tract tumors. Among these tumors about 45% occur from the duodenum’s third and fourth part.<span id="more-21"></span></p>
<p>When early diagnosis of colonic polyposis is done it has made the <strong></strong><strong>Duodenal Adenocarcinoma</strong> of the duodenum the main reason behind the death in patients of FAP. A continued 10 year study has revealed that the six patients among the 114 FAP patients died because of <strong><a href="http://duodenal.org/duodenal-adenocarcinoma/">Duodenal Adenocarcinoma</a> .</strong>  The rate of incidence found to be higher in those types of patients who had more advanced duodenal polyposis at start of the study.  The signs of this disease are not clear and specific. There can be signs like abdominal pains, weight loss, bleeding, jaundice or obstruction.</p>
<p>There is a different diagnosis for patients who show epigastric discomfort.  The starting findings remain oesophago-gastroduodenoscopy and distinguished studies that can generally display the site, severity, and the lesion’s length. Proximal tumors can be found by oesophago gastro duo-denoscopy but the more distal tumors are generally found with the help of radiology-computed tomography, contrast studies and now PET.</p>
<p>In biopsy –confirmed cases there is a need of computed tomography for treatment planning and staging. When mucosal biopsy is taken while upper endoscopy a diagnostic dilemma comes up that shows inflammation. This can show the problem of peptic ulcer disease like bile duct and carcinoma of the <strong></strong><strong>Duodenal Adenocarcinoma </strong>or lesions such as gastro-intestinal stromal tumours or lymphomas.</p>
<p>Through endoscopic ultrasound measuring lesion’s thickness can be done which then will allow a more detailed biopsy of the lesion. Otherwise if there is no scope to pass then it can be good to opt for PET or computed tomography. If there is a presence of a greater volume of lesion then it shows the necessity for an operation. To get a final diagnosis intra-operative frozen section can be needed.</p>
<p>The only option for the tumor then remains of <a href="http://duodenal.org/duodenal-switch-surgery/">surgery</a> only.  The Whipple’s or radical pancreatic oduo-denectomy operation is an old type of operation but still it is being considered sometimes today also for the treatment of tumors  which are in <strong></strong><strong></strong><strong>Duodenal Adenocarcinoma</strong> first and second part.  In the third and fourth part many doctors are of the opinion to give duodenal segmentectomy treatment. But in both the whipple’s and duodenal segmentectomy treatment shows any difference in rate of survival of patients from the classical treatments.</p>
